Medical Statement Form
You must be medically fit-to-dive, that is - you must not be subject to any medical condition which would prohibit you from diving. Please download the MEDICAL STATEMENT here and complete it with a pen and bring to the first pool session.
If you answer 'YES' to any of the questions on the form you must be medically cleared and signed off by your own doctor before you do any diving.
If you can answer 'NO' to all the questions then you just need to sign it.
Please bring the completed form to our first meeting or before your first dive.

Which Course?
Scuba Diver, Referral  or the Full Open Water Diver course ?
The Referral and Scuba Diver course are really just portions / sections of the complete PADI open water diver course. The course is modular and performance based therefore they do not ALL have to be completed at the same time or with the same instructor, you can progress from one section to another with different instructors, even in different countries - this is how the course works.
All 3 courses each have the 3 following elements of Knowledge Development, Confined Water (pool) dives and Open Water (outdoor) dives.

Knowledge development
You will study at home at your own pace, using the PADI Open water diver manual and Padi Open Water DVD.
The manual has full colour sections with diagrams and explanations that also outline what you will be doing during your course. It guides you through the information required to Scuba dive in comfort and safety. There are 5 easy to read chapters each with a quick quiz at the end to guide you through the knowledge you will need. The 5th Quiz is the final exam.
The DVD has live action video scenes which explains and demonstrates the dive principles and previews your confined water (pool) training and your open water dives.
Your Instructor reviews your quiz answer sheets and marks your exam and reviews any sections which need clarification and ensures you know how and when to use the information in actual diving circumstances.

Confined water (pool) sessions
There is a minimum of 5 pool sessions required to complete the PADI open water diver course during which you will learn the skills necessary to dive in safety under the supervision and direction of a professional PADI instructor.

Open water Dives
There are 4 open water dives required to complete the PADI open water diver course. These are conducted over 2 days at an outdoor dive location such as Capernwray, Blue Lagoon or Stoney Cove. On these dives you will demonstrate to the instructor the skills and procedures you learned during your pool dives, without the instructor demonstrating them to you.

SCUBA DIVER
This course consists of the first 3 knowledge development sessions, the first 3 pool dives and the first 2 open water dives of the Open Water Diver Course.
On completion you will be awarded the PADI Scuba Diver certification card and will be able to dive under the supervision of a PADI Divemaster, Assistant Instructor or Instructor to a maximum of 12 metres.
The Scuba Diver course option is ideal for people who are not yet ready to complete the full Open Water Course or for those divers for whom it will be more appropriate to continue diving under the supervision of a PADI Professional.

Scuba Diver is also suitable for those who only want to dive on a one-off special holiday or have not decided whether they want to continue with diving. If you decide to continue and complete the extra sections of the Open Water Course you can do this at a later date. If you want to gain the full Open Water Diver qualification you will have one year (12 months) to complete your knowledge development sessions 4 & 5 with final exam, final 2 pool dives and last  2 open water dives, this qualifies you to dive to 18 metres with another qualified diver ( without the supervision of a Padi Divemaster, Assistant Instructor or Instructor).
The Scuba Diver Qualification allows you to dive to 12m with a Padi Professional.

THE REFERRAL COURSE
This course consists of  5 knowledge development and final exam and 5 confined water (pool) dives. On completion of these sections you can then complete your 4 open water dives at any PADI dive center in the world.
The Referral Course option is favoured by people who have a holiday booked or are planning a holiday overseas . You can complete the knowledge development in your own time and pool dives here in the UK and then go diving almost straight away whilst completing your final 4 open water dives on holiday
It is also suitable in the event that something prevents you from completing a full open water course (like a cold or illness) the referral gives you credit for what you have done and allows you to complete the remaining portions of the PADI Open water course later.

OPEN WATER DIVER
The PADI Open Water Diver Course is the worlds most popular entry level diving course and incorporates 5 knowledge development sessions with a final exam, 5 confined water (pool) dives and 4 open water dives.
Once you have met all performance requirements and on completion of the final open water dive you will be awarded the PADI open water diver certification card which qualifies you to scuba dive with another qualified diver to a maximum of 18m.
